Westlake residents will be required to provide proof of residency. Primary Household Member must be 18 years of age and older. Once you have completed the Household Primary Person information, click "Add New Member" to add a person(s) to your household (dark button found at the bottom of this page next to CANCEL button). Anyone 18+ MUST reside at the same address as primary holder and provide proof. You will be unable to modify your account once it has been approved. All information is subject to review.
